Vermilion Parish Early Childhood Network | Pre-K Application Documents | Enrollment Requirements

You will need the following documents to complete to your child's application.

Our application is 100% online!

Child’s Birth Certificate. Must be 4 years old before September 30th.
Child’s Social Security Card.
Child’s current immunization record. Must have expiration date at the top of the page record.

CHILD RESIDENCY DOCUMENTS

TWO Residency documents must be within 60 days of application submission date.

First Document

Current electric bill where the child resides.

* NOTHING ELSE WILL BE ACCEPTED!

Second Document

Is the electric bill payee a parent on the child's birth certificate?

PROOF OF INCOME DOCUMENTS

EACH adult, 18 years or older, who resides in the household MUST provide ONE of the following:

Assistance Programs:

Does your child receive SNAP, Medicaid or Social Security Benefits?

If YES - you must provide SNAP/food stamp/Medicaid statement with child's name and current certification date, If a Social Security benefits recipient, must provide details page for the Pre-K child and all family members living in the household who receive benefits,

Is your child receiving foster care?

If YES - provide Signed Foster Care Placement Agreement by Department of Child & Family Services.

Is anyone in your household homeless?

If YES - provide Vermilion Parish Student Services Homeless verification papers.

Unemployed: Does any adult in the household currently not work?

If YES - must complete a 'Statement of No Income'. All forms located at the bottom of this page.

Employed, with Irregular Income: Does any adult in the household work seasonally, is self-employed, receive cash payments, or have an hourly rate or gross income that is different from one check stub to the other?

If YES - must complete 'Declaration of Irregular' form. All forms located at the bottom of this page.

Employed, with Consistent Income: Does any adult in the household work and receive pay stubs from their employer?

If YES - must provide TWO (2) consecutive dated check stubs from the last 2 months of employment with the same rate of pay on each stub.

**Pay stubs must be dated on after the other, with no gaps in dates between the two stubs.

**Personal Checks will not be accepted.

Or a signed employer letter on business letterhead

**Must include business name/address of employment, rate or pay, & the average number of work hours per week.

Tuition Information

Program eligibility is based on socio-economic status and will be calculated using the LA4 Program eligibility criteria. Since families that are above the PreK program's income requirement are ineligible for LA4 program funding, they can be offered a tuition paid seat. Tuition student placement will be prioritized using the parent's responses on the child's health and development portion of the application.

Monthly tuition payments will ONLY be accepted by bank draft. Default payments can result in removal from the program. Our program tuition is $3,250.

Payment options include:

Installment Payments: Tuition is divided into 10 installments from August to May: 10 payments of $325 per month. The installment payment amount is not impacted by the number of school days within a month.
Paid in Full: To be paid in August as a money order or cashier’s check payable to Vermilion Parish School Board.
